Targeted Case Management
Our primary goal of mental health case management is to obtain the much needed services for our client such as medical, social, educational, mental health, housing, insurance, employment, financial and other services, that will allow them to function as independently as possible in their communities. Our case managers coordinate the provision of quality treatment and support services to advocate for their clients' needs.
